Personalized care for your rehabilitation needs
Inpatient rehabilitation is a stepping stone towards home for persons with physical, cognitive and/or speech/language impairments due to illness, injury, disease or other causes. Inpatient rehabilitation is appropriate for patients who can tolerate three hours of intensive therapy per day and have a medical need to be closely followed by a doctor.
Who can benefit from inpatient rehabilitation?
Individuals who will benefit from this setting include those who are recovering from severe illness or injury, accompanied by a significant loss of function. Some common diagnoses include:
- Stroke
- Spinal cord injury
- Amputation
- Major multiple trauma
- Fracture of femur (hip)
- Traumatic brain injury
- Neurological disorders (e.g., multiple sclerosis and Parkinson's disease)
- Complicated total joint replacements
- Various medical diagnoses with loss of function
Therapy
The different types of therapy offered on the rehabilitation unit are designed to help patients increase their independence while meeting therapy goals to be safely discharged home. Every patient on the unit receives physical therapy and occupational therapy.
Many patients may also receive speech therapy if difficulties in communication, problem solving, memory and/or swallowing are identified. Recreational therapy is also available to each patient.
Rehabilitation care team
The rehabilitation care team works closely with each patient to restore function and increase independence. The team meets weekly to discuss and update the individual care plan for each patient.
Depending on the patient's specific needs, the rehabilitation team may consist of:
- Rehabilitation doctor (24/7 medical coverage provided)
- Rehabilitation nurses
- Physical therapist
- Occupational therapist
- Speech-language pathologist
- Respiratory therapist
- Registered dietitian
- Diabetic specialist
- Wound care specialist
- Pharmacist
- Case manager
Team members interact closely with the patient and his or her loved ones, communicating the treatment plan and encouraging input. The team may make arrangements to do home safety evaluations prior to discharge to ensure all barriers to discharge are identified and resolved prior to discharging the patient.
