    • Cardiac Rehabilitation is an outpatient program that helps you work toward lifestyle improvements if you have coronary heart disease, angina, or are recovering from a heart attack, heart surgery, stent placement or other heart conditions. Each session will include a progressive exercise program and education training tailored to your specific needs and health condition.
    • Pulmonary Rehabilitation is an outpatient program designed for adults who are limited in their everyday activities due to lung disease. Each session will include a progressive exercise program and education training tailored to your specific needs and health condition.
    • Physical Therapy is designed to promote, maintain, and/or restore health through physical examination, diagnosis, and rehabilitation treatments.
    • Occupational Therapy is designed to promote the patient’s ability to perform common daily tasks critical to their quality of life. Specific therapeutic exercises and menial therapy are used to enhance basic/fine motor skills and dexterity.
    • Speech Therapy provides techniques designed to stimulate cognitive, communicative, and physical skills related to verbalization and swallowing that are critical to quality of life.

Public Notice

Ascension Saint Thomas Three Rivers has applied for Federal Emergency Management Agency (FEMA) Hazard Mitigation Grant Program (HMGP) funding through the Tennessee Emergency Management (TEMA), as a sub-recipient. Under the National Environmental Policy Act (NEPA), federal actions must be reviewed and evaluated for feasible alternatives and for social, economic, historic, environmental, legal, and safety considerations. Under Executive Order (EO) 11988 and EO 11990, FEMA is required to consider alternatives to and to provide a public notice of any proposed actions in or affecting floodplains or wetlands. EO 12898 also requires FEMA to provide the opportunity for public participation in the planning process and to consider potential impacts to minority and low-income populations. This notice may also fulfill requirements under Section 106 of the National Historic Preservation Act (NHPA). Funding for the proposed project will be conditional upon compliance with all applicable federal, tribal, state, and local laws, regulations, floodplain standards, permit requirements and conditions. 

PROPOSED WORK AND PURPOSE: Ascension Saint Thomas Three Rivers respectfully requests funds to upgrade the generator and transfer switches. Ascension Saint Thomas Three Rivers will take ownership and be responsible for post-project site maintenance and inspections.
