MyGULFCare
The MyGulfCare program at Ascension Sacred Heart Emerald Coast Hospital in Miramar Beach, Florida, is designed to help you manage your chronic health condition by providing personalized education and assistance to ensure the highest possible quality of life.
If you have one of the following health conditions:
- Chronic obstructive pulmonary disease
- Diabetes
- High blood pressure
- High cholesterol
- Obesity
- Pneumonia
We are here to help!
As a MyGulfCare participant, you will have access to a care manager and patient navigator. Our care managers are licensed registered nurses that are here to discuss your health conditions with you and assist in chronic disease self-management. Our goal is to assist you in setting and meeting goals that will improve your health and your health outcomes.
Services we provide:
- Chronic Disease management
- Help with social barriers
- Medication assistance
- Assistance in finding a doctor or specialist
Eligibility
MyGulfCare services are available to residents of Walton County, Okaloosa County, and West Bay for people living with COPD, diabetes, high blood pressure, high cholesterol, obesity and persistent pneumonia.
