About Valerie C Rosen, MD
Dear Patient/Family Member,
Thank you for trusting us with your care at Ascension Medical Group Seton Behavioral Health, located at 1301 W. 38th Street, Suite 700, Austin, TX 78705. We are writing to inform you that Dr. Valerie Rosen, M.D. will no longer be providing care through our clinic, effective June 20, 2025.
Dr. Rosen has served our community for over a decade, in our outpatient clinic and as Medical Director of the Restore Intensive Outpatient Program (IOP) for PTSD, which concluded operations on June 1, 2025. As she transitions to a new role with Dell Medical School, we remain committed to supporting your ongoing care. Dr. Rosen has attempted to reach all of you by phone, and over the coming weeks, our care team will work closely with you to ensure a smooth and supportive transition.
Options for Continued Care: If you have an upcoming appointment with Dr. Rosen after June 20, 2025, or need to schedule follow-up care, our team can help reschedule you with another provider at this location. Please see the back with important information about our resident clinic. If you prefer to transition your care to a provider outside of Ascension Medical Group, we are also available to assist with that process.
Medical Records Access: Your medical records will remain securely with Ascension Medical Group. If you would like to request a transfer of your records to an outside provider, we are available to assist you. If you have any questions or need support during this transition, please don't hesitate to contact our office. We can be reached at (512) 324-3380.
We thank Dr. Rosen for her dedicated service and her leadership of the Restore Intensive Outpatient Program, which supported close to 500 patients across 40 cohorts. We wish her all the best in her next chapter.
Our highest priority is to ensure that you continue to receive compassionate, uninterrupted, and high-quality care. Thank you for allowing us to be part of your healthcare journey.
Sincerely, Ascension Medical Group Seton Behavioral Health
Ascension Medical Group (AMG) Seton Behavioral Health Adult and Geriatric Psychiatry Training Clinics Clinic Guidelines & Patient Responsibilities
Background: Our psychiatry trainees are a dedicated and talented group of physicians who are eager to provide you the highest quality of care. For background, residents and fellows in psychiatry are medical doctors who are completing four or five years of specialty training in psychiatry. They are employees of the hospital and have a medical license from the state of Texas. When residents and fellows complete training in our accredited program, they are eligible to become board-certified psychiatrists and geriatric psychiatrists.
Treatment Modality (in-person vs. telehealth): In person visits are required for all transfer visits (transferring care to a new resident provider) at the start of the new academic year and every 6 months of treatment.
If your treatment occurs via telehealth, you must be a resident of Texas and physically located in the state at the time of the visit and must be willing to arrive in person if it becomes medically necessary.
All visits in the geriatric training clinic are in person without exceptions.
Length of Treatment and Care Guidelines: Our clinic functions predominantly in a consultative model with very limited capacity to follow patients longitudinally. This means that we usually make recommendations to your primary care physician or refer you to a provider in the community and only see you for a limited number of psychiatry appointments.
If you choose not to continue care with your resident physician, please be aware that we do not transfer care to physicians within the clinic. In that case, we will provide referrals indicating where you can seek care elsewhere. With your consent, we will share medical records with any outside provider you choose.
Sincerely, Ascension Medical Group Seton Behavioral Health
